Break through – improve late claims results

To achieve improved results follow these important procedural guidelines:

Determine deadline

Determine deadline for responding to forms/notices sent by state agency:

  • Claim forms/notices
  • Determinations/decisions
  • Charge statements
  • Hearing notices

Four days or less...

  • Fax or overnight all claim/hearing forms/notices to your UC eXpress Claims representative for completion or response.
  • If manager completes claim form/notice send original to state agency and send a copy to your UC eXpress Claims Representative.

Five days or more...

  • Claim Forms/Notices – Send separation information with any additional documents, warning notices and comments to your UC eXpress Claims Representative for completion.
  • Hearing Notices – Fax or call your UC eXpress Claims Representative.
  • Determinations/Decisions/Charge Statements – Mail to UC eXpress.
